New Patient Registration
The Practice provides general medical services for patients within our practice boundary. New patients should check with reception to see if they fall within this area.
All UK residents have the right within the NHS to be registered with a doctor. We are pleased to register anyone who lives within the practice area. We will need the following information on the form.
- Full name, including previous names
- Date and place of birth
- Last address
- New address and post code
- Last name and address of your previous Doctor
- NHS number - if not known (it is not the same as your National Insurance number) enquire from your previous doctor before attending to register.
Registration Forms
You will need to complete an NHS (GMS1) Registration Form for each member of your household when you register in person and the patient health questionnaire below, please print and hand these in to reception.
You will need to print a separate form for each member of your household who wishes to register as a patient with us.

New Patient Health Check
Every patient registering with the practice will have the opportunity of a new patient consultation. The appointment gives you a chance to meet us, have your health checked and bring to light any ongoing medical problems or important past medical history, including continuing your medication.
Your appointment may be with a practice nurse.
We will ask you about the following:
- Your past medical history (important illnesses and operations)
- Ongoing medical problems
- Medication
- Family history
- You will have your blood pressure taken and your urine will be tested. Occasionally other examinations or tests may be done if needed.
We will then send you a letter to confirm you have joined our practice list.
Changes to Personal Details
You should notify us of any changes to your name, address or telephone number. This will avoid wasted hospital appointments, confusion and delays.
